I've had the exact same problems. Dealer changed the intercooler, reflashed computer. Problem still existed. Then I started using the Motorcraft cetane booster. No problems in 24,000 miles since using. I pour about 3 ounces in every fill up. I think a lot of the problems are fuel related.
